Nomination for the First Annual Healthcare HR or Educator Excellence in Leadership Award

Purpose: To recognize an active TSHHRAE member who is in good standing and whose significant contributions advance the healthcare Human Resources profession.

NO SUBMISSION WILL BE CONSIDERED UNLESS ALL STEPS IN THE PROCESS ARE FOLLOWED

Selection Criteria:
A committee from the TSHHRAE Board will review all nominations and select award recipient based upon the following criteria:

1. The nominee provides a mechanism for the interchange of ideas and dissemination of information relative to healthcare human resources or education such as collaboration with other HR groups and/or community entities such as schools, clinics, civic organizations, etc.

2. The nominee promotes education and other professional activities dealing with HR or education administration and current healthcare issues such as serving as a role model or mentor.

3. The nominee shares knowledge and expertise through presentations, seminars and publications.

Award: Award recipient will receive the following recognition:

· Congratulatory letter from TSHHRAE President with a copy to the award recipient’s CEO & immediate manager as well as local press releases to the media.

· Recognition and award presented at the TSHHRAE Annual Meeting and/or conference

· Recognition and award presented at the Texas Hospital Association Leadership Conference

· Recognition in the TSHHRAE quarterly newsletter

· Recognition on the TSHHRAE website

· Recognition in Texas Hospital’s quarterly magazine

Submissions will be limited to double space, 12 font size, no more than one page per criteria. No additional attachments will be considered. Submissions must be received no later than September 23, 2008, by mail, fax or email. Fax or email is preferable.
